how many trips to the mound before a coach needs to make a change ? do they have to cross the line for it to count as a visit?

Scenario: You have a starting pitcher and it is inning 1. - You may visit the circle twice. On the third visit of the inning you must change the pitcher.
Scenario: You have a starting pitcher and it is inning 3. - You have a total of two visits to the same pitcher. On the third visit you must change the pitcher.
Rule: You have two defensive visits per game where you do not have to change the pitcher. On the third defensive visit, you must change the pitcher. Most coaches do not realize that it is a defensive visit, not necessarily a visit to the circle. And a visit is charged when a time-out is granted by the umpire. It has nothing to do with where the granted coach is on the playing field.
Rule continued: You have an unlimited number of defensive visits. After the second defensive visit of the game, each visit granted will require the coach to change the pitcher. This is another rule that is not understood well. When you are forced to change the pitcher, you do not get two new visits. It is total visits per game, not per pitcher.
